Forms for New Patients to our Midtown NYC Office

The following documents will assist you in expediting the check in process for your appointment. We would ask that you please print and complete the following forms and bring them with you to your appointment. Thank you for your cooperation. To open the files, you'll need software like Adobe Reader and Word.

Instructions:

Click on the link below to access the New Patient Form. After you click the link, a browser window will open. After it opens, click on the 'PRINT' icon on your Browser. After the form prints, fill it out and bring it into our office. Completing the New Patient Information Form will save you time on your initial visit!

What else should bring:

• Your insurance card along with another form of I.D. (i.e. - Driver's License, State ID card, Military ID)
• If you visited a podiatrist before, please bring any available X-Rays, MRIs or other pertinent medical records, if you have them.
• Information about prescription medication which you may be taking
• Cash or Checks for your Co-Pay
• Make a list of your symptoms and/or questions. (Will be helpful for you as patients often forget important questions they wanted to ask the doctor)
• Do you need a referral? Make sure you check with your insurance provider before your appointment.
• If you have special needs, make sure you inform your NYC Podiatry Center of Excellence before your appointment so that we can be sure to accommodate you.
• If you are encountering a problem while exercising and/or walking, bring the shoes that you typically wear while doing this activity with you to your appointment.

Methods of Payment

Your Health Insurance Plan should cover your visit, minus any deductible, co-insurance or co-pay.

Here's a list of accepted health insurance plans that we are in-network with.

If you are not covered by any Health Plans, no problem. We accept Cash, Check or Credit Cards and offer competitive rates.
